Abstract:

Objective:To analyze the efficacy and safety of thrombus aspiration combined with intracoronary injection of tirofiban during percutaneous coronary intervention (PCI). Methods: PubMed, Embase, Cochrane Library, CNKI, Wanfang Database, Weipu Database (VIP), and China Biomedical Literature Database were searched in computer from the establishment of the library to the publication of the literature in November 2022. Quality was assessed by two investigators and statistically analyzed using Revman 5.3 software. Results: A total of 7 randomized controlled trials with a total of 1 152 patients were included, including 585 in the combination therapy group and 567 in the PCI alone group. Among them, 708 were males and 444 were females. According to the results of meta-analysis, the ST segment drop in patients treated with acute myocardial infarction (AMI) in the combination group was greater than 50% (RR=1.23, 95%CI 1.14~1.33, P<0.01), postoperative TIMI grade 3 blood flow (RR=1.20, 95%CI 1.14~1.27,P<0.01), left ventricular ejection fraction( LVEF)( MD=3.32, 95%CI 1.51~5.13,P<0.01), left ventricular end diastolic diameter( LVEDD)( MD=-2.22, 95%CI -4.17~-0.28,P<0.05), creatine kinase isoenzyme( CKMB) peak (MD=-111.85, 95%CI -116.43~-107.27, P<0.01) were better than PCI alone, and the incidence of major adverse cardiovascular events (MACE) in the combination therapy group (RR=0.54, 95%CI 0.38~0.75, P<0.01) was lower than PCI alone. Conclusion: The efficacy and safety of the combination therapy group were better than those in the PCI alone group.
