    Association Study of TLR9 Gene and Its Polymorphism at -1237T / C Site with Cardiovascular Disease

    Objective:To explore the relationship between toll-like receptor 9 (TLR9) gene and cardiovascular disease by bioinformatics analysis of TLR9 gene and its -1237t / C polymorphism. Methods:The physical and chemical properties,single nucleotide polymorphism(SNP) sites,associated gene interaction and GO function annotation of TLR9 were analyzed by bioinformatics.The genomic DNA of peripheral blood was extracted from 189 plasma samples (including 68 patients with cardiovascular disease and 121 healthy controls). After gene amplification,the amplified products were digested,typed and sequenced,the correlation with cardiovascular disease was statistically calculated. Results:TLR9 is a hydrophilic basic protein with four nuclear localization signals and several potential linear B cell epitopes. The secondary structure of the protein is mainly α helix. As a hub protein,TLR9 interacts with 10 proteins such as MYD88,TBK1 and CD40,and is mainly enriched in Toll-like receptor signal pathway and inflammatory response signal pathway. The distribution frequencies of TT and TC genotypes were 71.90% and 28.10% in the healthy control group,47.06% and 52.94% in the disease group,respectively. There was statistical significance between the two groups. Conclusion:TLR9 gene and its 1237T / C polymorphism are closely related to cardiovascular diseases.

    Gender Differences and Characteristics of Coronary Artery Disease in Patients with Early Onset Acute STEMI Treated by Direct PCI

    Objective:To investigate the gender difference of direct percutaneous coronary intervention in patients with early-onset acute ST-segment elevation myocardial infarction(STEMI). Methods:A total of 116 patients with early-onset acute STEMI who received direct PCI in the Department of Cardiovascular Medicine,Hebei General Hospital from September 01,2018 to December 31,2019 were selected as the study subjects,including 95 male patients and 21 female patients. The clinical data,surgical data,examination indicators and complications during hospitalization of 2 groups were collected and analyzed,and the differences were evaluated. Results:Compared with the male group,the female group was older,with higher score of GRACE and CRUSADE,higher ratio of type 2 diabetes,higher level of HDL-C and GLU,and higher risk of nosocomial adverse cardiovascular events (MACE),lower levels of hemoglobin,eGFR,and uric acid,and longer time from admission to balloon dilation (D2B time) and average length of hospital stay( P<0.05). The proportions of stent diameter and smoking history in male group were higher than those in female group (P<0.05). Conclusion:Among patients with premature acute STEMI undergoing direct percutaneous coronary intervention,female patients were at higher risk of nosocomial adverse cardiovascular events and mortality.

    Meta-Analysis of the Clinical Efficacy of Xiaoyao Powder on Insomnia

    Objective:A meta-analysis was conducted to compare the clinical efficacy of Xiaoyaosan and Western medicine in the treatment of insomnia. Methods:The databases included CNKI,WanFang,VIP,CBM,Pubmed,Embase,Web of Science and Cochrane Library,et al. The quality evaluation was conducted for eligible clinical randomized controlled trials literature which meet inclusion and exclusion criteria. The effective rate,adverse reaction rate and Pittsburgh sleep quality index(PSQI) of related studies were analyzed by Revman 5.3 and Stata 16.0. Results:A total of 540 literature studies were screened,and 10 of them (including 693 patients) were finally selected. According to meta-analysis,compared with the west medicine group,the Xiaoyaosan group has a higher effective rate (RR=1.27,95%CI(1.16,1.38),P<0.00001) ,a lower clinical adverse reaction rate (RR=0.38,95%CI(0.21,0.70),P=0.002) and a more significant decrease in the PSQI (MD=1.43,95%CI(0.13,2.72),P=0.03). Conclusion:Compared with conventional western medicine,the efficacy and the safety of insomnia treatment as well as the sleep quality of insomnia patients are significantly improved by Xiaoyaosan. However,due to the low quality of the included randomized controlled trials,a large number of scientifically reasonable clinical studies are urgently needed for further verification.

    Thin-layer Separation and Qualitative Identification of the Alcohol Extract of Taohong Siwu Decoction

    Objective:Thin layer separation and qualitative identification of alcohol extract of Taohong Siwu decoction. Methods:Thin layer chromatography (TLC) was used to observe the development of chemical components in the alcohol extract of Taohong Siwu decoction,and iodine vapor was used to perform color rendering on the spots,and the thin layer plate was placed in a thin-layer scanner for scanning. The absorption spectra and retardation factor (Rf)values of the spots were recorded and compared with the reference substances. The silica gel G thin laminate was used as the stationary phase,and n-butanol-acetic acid-water (4∶1.5∶5,upper layer) was used as the developing agent,and examined under UV lamp (365 nm). Results:A total of 5 spots were isolated from the alcohol extract of Taohong Siwu decoction,among which 3 spots were qualitatively identified as hydroxyl safflower yellow A (from Carthami Flos),paeoniflorin(from Paeoniae Radix Alba),and ferulic acid (from Angelicae Sinensis Radix and ChuanXiong Rhizoma). The other 2 spots were not detected in the six single drugs of Taohong Siwu decoction. Conclusion:TLC can be used to separate the chemical constituents from the alcohol extract of Taohong Siwu decoction,and on this basis,new constituents can be further identified. The separation method is simple,rapid,stable,and feasible.

    Leech is a traditional Chinese herbal medicine. Hirudin is a natural polypeptide extracted from leeches and fresh saliva,which has anticoagulant effects and is widely used clinically,but not all varieties of leeches contain hirudin in the body. This article reviews the anticoagulant active substance components contained in several common leeches and the extraction and purification methods of hirudin,aiming to provide reference and reference for in-depth research on the method of efficient and rapid extraction of hirudin.

    Migraine is a recurrent chronic neurovascular headache. It is one of the common primary headache in clinic. It affects patients’ daily life and work to a great extent,hinders the development of social productivity and brings huge economic burden to individuals and society. At present,a large number of studies at home and abroad have found that migraine is related to right-to-left shunt (RLS),but the specific principle is still under study. Studies have shown that the incidence of white matter lesions (WML) in migraine patients is three times higher than that in normal people,and its pathophysiological mechanism has not been clarified. The results of recent studies suggest that the prevalence of patent foramen ovale (PFO) in migraine patients was significantly higher than that in normal people,especially migraine with aura (MA). Whether there is a relationship between white matter lesions and right to left shunt in migraine patients, and the pathophysiological mechanisms between them are not clear,which is worthy of further study.

    Cerebral stroke is the most common cause of death in China. It has the characteristic of high incidence rate,high disability rate,high recurrence rate,high mortality rate and high financial burden. It seriously endangers the health of our people. Hypertension is a major risk factor for cerebrovascular disease. In order to prevent target organ damage effectively,maintain vascular integrity,reduce incidence rate and mortality,it is necessary to maintain the stability of blood pressure within 24 hours. Morning blood pressure surge (MBPS) refers to the phenomenon that blood pressure rises sharply in the early morning. In recent years,many studies have found that MBPS is closely related to cerebral stroke. How to better control MBPS,reduce target organ damage,reduce the occurrence of cerebral stroke,is the issue in the field of cerebrovascular disease.

    Hypertension is one of the most common cardiovascular and cerebrovascular diseases in the world,and its understanding has gradually penetrated into the fields of centra and inflammatory factors. Under the same living conditions,which groups are more likely to induce hypertension and the mechanism of its induction have become the most important research direction in the primary prevention of hypertension. Generation of a high-fat diet during pregnancy and lactation,can raise the offspring of central nervous system related to blood pressure,increase high blood pressure,central sensitivity,adults are more likely to form the epigenetic metabolic syndrome,compared with other groups in exposure to the risk factors of sympathetic activation,such as high fat,high salt,high sugar intake,and so on and so forth,are more likely to appear blood pressure of adverse reactions. This article reviews the mechanism and significance of central sensitization of hypertension in early life.

    Diabetes mellitus is a metabolic disease with abnormally high blood glucose as the main clinical manifestation. The absence or relative insufficiency of insulin is the main mechanism of diabetes mellitus. Currently,the mainstream treatment methods are to increase the body’s glucose use and enhance insulin sensitivity and insulin volume to control blood glucose.Sodium glucose co-transporter 2 inhibitor is a new oral drug for the clinical treatment of type 2 diabetes mellitus. Currently,as a second-line adjuvant in the clinical treatment of diabetes,it is different from the hypoglycemic mechanism of traditional insulin and has attracted attention.With the application of clinical treatment,the clinical researchers found SGLT2 inhibitor not only in the treatment of type 2 diabetes show strong hypoglycemic effect,at the same time,in patients with diabetes with or without cardiovascular,critical organs such as liver and kidney damage also have additional protection,SGLT2 inhibitor organ protection mechanism is not clear,however,is associated with a significant side effects,with the deepening of research,the function and mechanism of SGLT2 inhibitor is further found. This paper mainly summarized the role of SGLT2 inhibitor in lowering blood glucose,improving cardiocerebrovascular disease,kidney injury protection,anti-tumor and other aspects,as well as the potential organ protection mechanism.

    Research Progress of Atorvastatin in the Treatment of Diabetes Mellitus Complicated with Coronary Heart Disease

    The research progress of atorvastatin in the treatment of type-2 diabetes mellitus complicated with coronary heart disease was reviewed. The impacts of different doses of atorvastatin on curative effect were introduced and the short-term efficacy of atorvastatin,simvastatin and rosuvastatin was compared and the first two former was better than that of the last. Atorvastatin in combination with an antihypertensive agent such as telmisartan,benazepril, lisinopril,or vigliptin enhances efficacy and improves safety. A combination of atorvastatin with Yangyin Huoxue decoction,or Dengzhan Xixin injection,or Xuefu Zhuyu tablet,or Jiangzhi Tongmai Capsules,or Tongxinluo capsule or Ezetimibucan also enhance the efficacy,improve safety and reduce the incidence of adverse events.
