不同剂量喷他佐辛用于无痛胃镜检查术的效果观察

Observation of the Effect of Different Doses of Pentazocin for Painless Gastroscopy

摘要:

目的: 探究不同剂量喷他佐辛在无痛胃镜检查中应用效果。方法:选取2022 年1 月~ 2023 年1 月于河北 北方学院附属第一医院进行无痛胃镜检查的患者200 例。采用随机数字表法将所有患者分为A 组(喷他佐辛0.2 mg·kg-1)、B 组(喷他佐辛0.5 mg·kg-1)、C 组(喷他佐辛0.8 mg·kg-1)、D 组(对照组:舒芬太尼0.08 ug·kg-1)四组,每组50 例。观察指标:采用非插管患者行为疼痛量表(behavioural pain scale non-intubated,BPS-NI)评分,比较四组患者镇 痛效果。比较术前(T0)、胃镜跨越声门到达食管上端(T1)、唤醒时(T2)四组患者的平均动脉(mean arterial pressure, MAP)压和心率(heart rate,HR),记录苏醒时间、恢复室(postanesthesia care unit,PACU)滞留时间、丙泊酚用药总 量。比较术中和术后不良反应的发生情况。结果:与D 组相比,A 组镇痛效果差(P<0.05),丙泊酚用量、苏醒时间和 PACU 滞留时间均高于D 组。C 组丙泊酚用量低于D 组,但苏醒时间和PACU 滞留时间和不良反应发生率明显高 于C 组(P>0.05)。B 组BPS-NI 评分和镇痛成功率与D 组相当(P>0.05),但术后不良反应发生率低于D 组,血流动 力学稳定性优于D 组。结论:0.5 mg·kg-1 的喷他佐辛在无痛胃肠镜检查中应用效果最佳,值得临床推广。

Abstract:

Objective: To explore the effect of different doses of pentazocine in painless gastroscopy. Methods: A total of 200 patients who underwent painless gastroscopy in the First Affiliated Hospital of Hebei North University from January 2022~January 2023 were selected. All patients were divided into four groups: group A (pentazocin 0.2 mg·kg-1), group B (pentazocine 0.5 mg·kg-1), group C (pentazocine 0.8 mg·kg-1) and group D (control group: sufentanil 0.08 ug·kg-1), with 50 cases in each group. Observation indicators: The behavioural pain scale non-infused (BPS-NI) score was used to compare the analgesic effect of the four groups. The mean arterial pressure (MAP) pressure and heart rate (HR) of the four groups were compared before operation (T0), gastroscopy across the glottis to the upper esophageal end (T1), and at the time of awakening (T2), and the recovery time, recovery ventricular retention time and total amount of propofol were recorded. The occurrence of intraoperative and postoperative adverse reactions was compared. Results: Compared with group D (control group), group A had worse analgesic effect (P<0.05), and the dosage, recovery time and retention time of PACU were higher than those in group D. The dosage of propofol in group C was lower than that in group D, but the recovery time and retention time and incidence of adverse reactions in group C were significantly higher than those in group C (P>0.05). The BPS-NI score and analgesic success rate of group B were similar to those of group D (P>0.05), but the incidence of postoperative adverse reactions was lower than that of group D, and the hemodynamic stability was better than that of group D. Conclusion: Pentazocine at 0.5 mg·kg-1 has the best effect in painless gastrointestinal endoscopy, which is worthy of clinical promotion.
