偏头痛患者心脏右向左分流与脑白质病变相关性研究

摘要:

目的:研究偏头痛患者心脏右向左分流(right-to-left shunt,RLS)与脑白质病变(white matter lesions,WML)的相关性。方法:选取自2020 年9 月至2021 年11 月就诊于河北北方学院附属第一医院神经内科门诊,临床诊断为偏头痛且符合纳入标准的152 名患者。记录患者一般资料,对所有入组患者均进行发泡实验(contrast-enhanced transcranial doppler,c-TCD)及头颅磁共振(magnetic resonance imaging,MRI) 检查。依据头颅MRI 将患者分为WML+ 组与WML- 组。采用SPSS25.0 软件包进行统计分析,查看RLS 与WML 之间有无相关性。结果:WML+ 组中RLS(+)共50 例,WML- 组RLS(+)共25 例,两组间比较有无分流差异,具有统计学意义(χ2=4.016,P=0.045);右向左分流不同级别中,脑白质病变评分差异具有统计学意义(Rho=0.208,P=0.010)。结论:偏头痛患者的WML 发生率与RLS 相关,RLS 级别越高,WML 越严重。

Abstract:

Objective:To study the correlation between right-to-left shunt (RLS) and white matter lesions (WML) in patients with migraine. Methods:152 patients who were diagnosed as migraine and met the inclusion criteria were selected from the neurology clinic of the First Affiliated Hospital of Hebei North University from September 2020 to November 2021. The general data of patients were recorded and the migraine scale was registered. All patients were examined by contrastenhanced transcranial Doppler (c-TCD) and magnetic resonance imaging (MRI). According to the results of cranial MRI,the patients were divided into WML+ group and WML- group. Spss25.0 software package for statistical analysis to check whether there is correlation between RLS and WML. Results:There were 50 cases of RLS(+) in WML+ group and 25 cases of RLS(+) in WML group.The difference between the two groups was statistically significant( χ2 = 4.016,P = 0.045).The scores of white matter lesions in different grades of right-to-left shunt were statistically significant (Rho=0.208,P=0.010). Conclusion:The incidence of WML in migraine patients is related to RLS. The higher RLS level is,the more serious WML is.
