摘要：

慢性肾衰竭（chronic renal failure, CRF）作为多种慢性肾脏疾病进展的终末期阶段，其病理机制涉及肾 纤维化、炎症反应等复杂过程，是临床疑难病证之一。国医大师颜德馨教授创立的“衡法”理论以“调气血、平阴 阳”为核心，强调通过平衡机体的气血津液运行防治疾病。现代研究表明，巨噬细胞极化状态（M1/M2 型）失衡在 CRF 肾纤维化进程中起到关键作用，而衡法方药（固肾泄浊方）可能通过调节巨噬细胞极化、抑制炎症- 纤维化级 联反应，改善CRF 病理损伤。该文从衡法理论核心内涵出发，结合巨噬细胞极化分子机制，系统探讨两者在CRF 辨治中的相关性，为中西医结合治疗CRF 提供新思路。

Abstract:

Chronic renal failure (CRF), as the end-stage of various chronic kidney diseases, involves complex pathological mechanisms such as renal fibrosis and inflammatory responses, making it one of the difficult clinical diseases. The "Hengfa" theory founded by National Master of Traditional Chinese Medicine Yan Dexin takes "regulating qi and blood, balancing yin and yang" as the core, emphasizing the prevention and treatment of diseases by balancing the circulation of qi, blood, and body fluids in the body. Modern studies have shown that the imbalance of macrophage polarization status (M1/M2 type) plays a key role in the process of renal fibrosis in CRF, and Hengfa prescription (Gushen Xiezhuo Decoction) may improve CRF pathological damage by regulating macrophage polarization and inhibiting the inflammationfibrosis cascade reaction. Starting from the core connotation of the Hengfa theory and combining with the molecular mechanism of macrophage polarization, this paper systematically discusses the correlation between the two in the diagnosis and treatment of CRF, providing new ideas for the integrated traditional Chinese and western medicine treatment of CRF.
