宏基因二代测序与传统检测方法在中枢神经系统感染中的对比观察

Metagenomic Next‐Generation Sequencing and Traditional Way Contrast in Centralnervous System Infectious Diseases

摘要:

目的:探讨宏基因二代测序技术对中枢神经系统感染精准诊疗的价值。方法:分析选取2019 年5 月~ 2021 年5 月在河北北方学院附属第一医院神经内科住院,且临床诊断为中枢神经系统感染60 例病例,收集患者的 脑脊液(cerebro-spinal fluid,CSF)和宏基因二代测序法(metagenomics next-generation sequencing,mNGS)相关资 料。结果:所选病例包括细菌感染42 例,病毒感染16 例,寄生虫感染2 例。脑脊液mNGS 对细菌性感染的诊断 阳性率为95.24%,对病毒性感染的诊断阳性率为68.75%,寄生虫感染的诊断阳性率为50%。mNGS 的曲线下面 积(areaunder the curve,AUC)为0.705,诊断价值中等,与AUC=0.5 的诊断参考线比较,P<0.05,差异有统计学意义。 结论:宏基因二代测序技术对中枢神经系统感染的诊断较敏感,在中枢神经系统感染的精准化诊断方面具有独特优势。

Abstract:

Objective:To assess the clinical value of metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid in diagnosis of neurological infectious disease. Methods:Patients who were clinically diagnosed with neurological infectious treated in Department of Neurology,Affiliated Hospital of Hebei North University from May 2019 to May 2021 were retrospectively analyzed intotal 60,including the data of mNGS and traditional laboratory test of pathogens. Results:It was including 42 bacteria infection,16 viral infection and 2 parasitic infection. The accurate rate of bacteria,viral and parasitic by mNGS was 95.24%, 68.75% and 50%. The AUC of mNGS was 0.705 and the diagnostic value was medium. Compared with the diagnostic reference line with AUC=0.5,the difference was statistically significant (P<0.05). Conclusion:ThemNGS is more sensitive evaluating the neurological infectious. It has advantages in accurate diagnosis of neurological infectious disease.
